Yes, all services are confidential in accordance with HIPAA regulations. Information will not be shared without your written consent, except in cases of danger to self or others, or as otherwise required by law.

As with any medication, there is a potential risk of reaction. We closely monitor all patients for any signs of reaction during their treatment, as well as during the required observation period. Our clinicians have the required training to address emergencies. You will be educated on the typical post-treatment signs and symptoms to be aware of, so that you know how to alert your clinical team if you experience anything outside of what may be typical.

Sage Elevate is located in Tampa, FL and services the Tampa Bay Area. 

Our center is open from 8am-4pm daily. Patients are seen by appointment only.

Yes. All patients need a physician referral to receive treatment at Sage Elevate. We can assist with reaching out to your provider’s office to obtain an order and supporting clinical documentation including labs or required testing. Our qualified team of clinicians can work directly with your provider’s office for all additional documentation and testing if needed.

Our rooms are semi-private. At Sage Elevate, we understand the need for discretion and have accommodated our clinic to include sound proofing and partition walls in a serene, peaceful environment.

We offer amenities such as ipads with noise canceling headphones, high-speed WiFi, free snacks and coffee, blankets/eye coverings, white noise and leather recliners.

Yes! We have a dedicated Caregiver space and your family member or caregiver is welcome to accompany you during your scheduled appointment.

Coverage can vary based on your insurance plan, Sage Elevate will provide a detailed estimate about what is covered and any potential out-of-pocket costs. Sage Elevate has partnered with all major insurance companies to bring the best price possible for our patients. Please note we will work with patients to receive financial assistance if needed. We accept all insurances (except Medicaid) for patients being treated with Sage Elevate.

The Sage Elevate team will perform a benefits investigation to confirm that the treatment is covered by your insurance company. We believe in providing all patients with true financial transparency therefore we will not schedule an appointment until we have discussed your benefits and financial responsibility with you.  

Coverage for medication and administration can vary. Typically, insurance may cover these costs separately. We will provide detailed information based on the patient’s specific insurance policy.

Yes, before your appointment, we will provide a My Cost Estimate. This detailed estimate will include a breakdown of costs and information on any financial assistance programs you may qualify for.

For those with private insurance, many copay assistance programs are offered by pharmaceutical companies. We will assist you with the enrollment process to help reduce your cost of care.

Yes, if you have a Medicare-type plan, Sage Elevate will research available foundations that offer financial support based on your specific conditions or treatment.

Medicare Advantage plans (also known as Medicare Part C) often require prior authorization before receiving certain treatments.

While behavioral health therapy is categorized under Medicare Part B as a drug benefit, Medicare Advantage plans are managed by private insurers. These plans may have their own specific rules, coverage guidelines, and requirements that differ from original Medicare.

Please notify the Sage Elevate team as soon as possible if your insurance changes. If your insurance changes during your treatment, we will review the new coverage details and adjust the cost estimates and financial assistance options accordingly.
